Toilets in an Ohio bathroom remodel: the rough-in, the clearances, the flange and the flush
When a bathroom is remodeled the toilet is usually the fixture that does not move, because what holds it in place is already under the floor: the drain, the flange and the vent. This page sets out what the Ohio Plumbing Code requires around a water closet, what a rough-in is and why it has to be measured rather than assumed, what a new floor does to the flange, and what federal law and the Environmental Protection Agency say a flush may use. Every figure is attributed to the document it came from, and the figures that come from a manufacturer are named as that manufacturer's.

What stays, what moves, and why
A bathroom can be taken back to the studs and the toilet will usually end up over the same hole in the floor. That is not laziness. It is what sits under the fixture: a drain the code holds to a three-inch minimum where it serves a water closet, a flange anchored to the structure of the building, a vent already run in the wall, and a branch that has to fall at a set rate to reach the stack. Moving a toilet asks all four of those questions at once. Replacing one in place asks none of them.
Before any section number appears it is worth settling which book those numbers come from. The Residential Code of Ohio requires every dwelling unit to have a water closet, a lavatory, and a bathtub or shower, at section 306.1. It has no plumbing fixture chapter of its own: section 2501.1 hands the subject to the Ohio Plumbing Code, which shall govern the installation, testing and operation of the plumbing in the buildings it covers. Every section number below is therefore a plumbing code section.
Reading one of those sections takes two documents. The rule posted at the state's code site is the list of Ohio's changes to a chapter; the body of a section lives in the 2021 International Plumbing Code, which Ohio incorporates by reference and then modifies chapter by chapter. Both are cited at the foot of this page, and where Ohio changed something, this page says so.
Locally, the City of Columbus names the 2024 Ohio Plumbing Code as the code behind its plumbing inspections, and says required inspections apply to systems in both residential and commercial buildings. Other Central Ohio jurisdictions run their own departments, and a mailing address is not a jurisdiction.
The rough-in, and why it gets measured
The rough-in is the measurement that decides whether a toilet bought on a Saturday will sit down on the bolts already in the floor. American Standard's specification sheets define it as the dimension from the finished wall to the centerline of the toilet's outlet, in a note printed beside the roughing-in drawing. No agency or standards body was found publishing a definition of the term, so the definition on this page is the manufacturer's own.
Twelve inches is the common one. Kohler's Wellworth specification sheets call a 12-inch rough-in the standard one, on both the elongated and the round-front bowl. The other sizes are real products: American Standard publishes the same Cadet Right Height elongated toilet in a 10-inch and a 12-inch rough-in on one specification sheet, and publishes a compact elongated model designed to rough in at 14 inches. Kohler also sells a trap and skirt assembly whose stated job is converting standard 12-inch rough-in products to a 10-inch configuration.
What no source states is how common a short rough-in is, or in what kind of house. That claim is everywhere online and nowhere in a document worth citing, so this page does not date it or tie it to any city. What the sheets do establish is that 10, 12 and 14 inches are all made and all provided for, which leads to the same place: measure from the finished wall to the center of the closet bolts before buying anything, rather than assuming twelve.
What the code requires around a toilet
The clearances sit in one section, 405.3.1 of the plumbing code, and they are the numbers a layout lives or dies by. Ohio's rule for that chapter changes none of them. Its only change to the section is a single added sentence, effective October 15, 2025, telling the reader to see Chapter 11 of the building code for the minimum required dimensions of accessible fixtures. The figures below are Ohio's by incorporation.

Two of those are easy to confuse. The 15 inches is measured sideways from the middle of the bowl, and the section counts a vanity or any other obstruction the same as a wall. The 21 inches is measured forward, to a wall, a fixture or a door, and it is the one a vanity or a door quietly takes away. The compartment sizes in the last two rows describe an enclosed stall, mostly a public-building matter.
The Residential Code of Ohio adds its own instruction that fixtures be spaced in accordance with its Figure 307.1 and with the plumbing code. That figure is a drawing rather than text in the published rule, so every number here comes from plumbing code section 405.3.1, where they are written out.
The drain, the trap and the vent
The three-inch figure everybody quotes is real, and it is easy to look for in the wrong chapter. It is footnote a to Table 710.1(1) in the sanitary drainage chapter, and it says the minimum size of any building drain serving a water closet shall be 3 inches, whatever the fixture unit arithmetic would otherwise allow. At the fixture itself the code also accepts a 4-inch by 3-inch closet bend.
That fixture unit arithmetic is why adding a bathroom is a larger question than adding a toilet. In the code's fixture table a private water closet flushing 1.6 gallons counts as three drainage fixture units, and one flushing more than that counts as four. A whole bathroom group built around a 1.6-gallon toilet counts as five, and six where the toilet flushes more. A footnote to the same table forbids rating a water closet at a lower value unless the lower values are confirmed by testing, so a 1.28-gallon toilet is still counted on the 1.6-gallon row.
A toilet has no trap under the floor, because its trap is cast into the china. The code's rule that fixtures be separately trapped carries an exception saying it does not apply to fixtures with integral traps, and Ohio rewrote the first paragraph of that section while expressly keeping its four exceptions intact.
The vent is the part that surprises people. For most fixtures the code caps the developed length from the trap to the vent: Table 909.1 gives a three-inch trap twelve feet at one-eighth inch of fall per foot, and a one-and-a-half-inch trap six feet. The exception to that section says the distance is not limited for self-siphoning fixtures, such as water closets. Fixtures may also share one vent: the code permits any combination of fixtures within two bathroom groups on the same floor level to be vented by a horizontal wet vent.
What does limit how far a toilet can travel is fall. The code requires horizontal drain pipe from 3 to 6 inches to slope at not less than one-eighth inch per foot, so every foot the fixture moves from the stack is another eighth of an inch of depth to find under the floor. Where a fixture's waste has to be pumped rather than fall, the code requires it to comply with a named standard for pumped-waste fixtures. The flange, and what a new floor does to it
The plumbing code says the connection between the drain and a floor outlet fixture is made with a floor flange or a waste connector and sealing gasket. The flange is attached to the drain and anchored to the structure of the building, and the same section sets minimum flange thicknesses by material and requires closet screws and bolts to be copper alloy. The fixture is then secured to the floor or to the flange with corrosion-resistant screws or bolts, and the joint where it meets the floor is sealed. A wall-hung bowl is a different arrangement: the code requires a concealed metal carrier attached to the building's structural members, so no strain reaches the fixture connector.
Notice what is not in any of that. The code says nothing about where the flange sits relative to the finished floor. That relationship comes from the makers rather than the code: Oatey, which manufactures flanges, instructs that the bottom of the top flange be parallel with the finished floor height, and sells a spacer kit whose whole purpose is making up the difference when it is not.
That distinction is the fact behind a problem every retiled bathroom meets. A new floor laid over the old one raises the finished surface, and a flange that was level with the old floor is now below the new one; a floor taken back to the subfloor does the opposite. What is worth knowing in advance is that the flange is a code element anchored to the structure, while the height it wants is set by the instruction sheet in the box. How much water a flush uses
The number that governs a new toilet is federal, and it is not new. Federal law limits any gravity tank-type toilet manufactured after January 1, 1994 to 1.6 gallons a flush, under the federal energy conservation standards. When that standard arrived, and what an Ohio bathroom looked like before it, is the subject of the Ohio bathroom history page. The Environmental Protection Agency says older, inefficient toilets can use as much as six gallons per flush, the agency's wording about older fixtures in general rather than a statement about any one house.
Below the federal limit sits a label rather than a law. The Environmental Protection Agency says a WaterSense labeled toilet uses 1.28 gallons per flush or less, 20 percent less water than the current federal standard of 1.6 gallons per flush, and that only toilets completing third-party certification earn the label. The label is not a water figure alone: under the specification now in force, a toilet has to clear all the test media from the fixture in a single flush, in at least four of five attempts.
Which specification is in force is a live question, and the answer is not the newest document. The agency published Version 2.0 of the tank-type toilet specification in May 2024, and that document carries an effective date of July 1, 2025 on its own face. The agency then paused that effective date, and says Version 1.2 remains in effect until further notice. Quoting the newer document as the current one would be quoting a specification that has not taken effect.
Seat height, bowl length and the seat itself
The height people call comfort height traces to an accessibility standard written for public buildings. The ADA Standards for Accessible Design put a water closet seat at 17 inches minimum and 19 inches maximum above the finished floor, measured to the top of the seat, at section 604.4. The same section carries an exception usually dropped when the figure is quoted: in residential dwelling units, the height is permitted to be 15 inches minimum and 19 inches maximum. The standard itself already treats a home differently from a store.
It also says where it applies. The Access Board describes the ADA Standards as the scoping and technical requirements for places of public accommodation, commercial facilities, and state and local government facilities. At home the number is a sensible place to start rather than a rule to obey, which is how the grab bars and seats page treats the public-building heights for a bar as well.
Manufacturers publish a different measurement, and the two do not describe the same thing. American Standard's Right Height sheets list a 16 and a half inch rim height and describe it as being for accessible applications. Rim height is the china with no seat on it, and the accessibility standard measures to the top of the seat, so the two cannot be compared as though they were.
Bowl shape is a household's own choice. The code requires an elongated bowl, and a hinged open-front seat, only in public or employee toilet facilities, and Kohler's sheets state the same limit in their own words. The dimensional difference is a per-model fact rather than a general one: in Kohler's Wellworth line the elongated bowl measures 30 inches front to back and the round-front bowl 27 and three-quarter inches, both at the same 12-inch rough-in. That is two and a quarter inches in that one line, and both sheets note their dimensions are nominal.
What needs approval, and where
Two published rules bear on the paperwork. The Residential Code of Ohio exempts the removal and reinstallation of water closets from approval, provided the work does not replace more than one fixture or rearrange valves, pipes or fixtures, in the exemption list of its administration chapter: swapping a toilet in place is exempt statewide, and the word carrying the weight is rearrangement. The City of Columbus, separately, lists a kitchen or bath remodel among the projects that require a permit and plan review on its Typical Residential Projects sheet, where flooring, painting and cabinets require none. That list is Columbus's own, and every other Central Ohio jurisdiction publishes its own; what a permit covers in Columbus is the subject of the permit page, and which code applies and who inspects the work is the subject of the code page.
What is actually left to decide
Two smaller things travel with the fixture. The Ohio Plumbing Code requires a shutoff valve on the fixture supply to each plumbing fixture other than bathtubs and showers in one, two and three family homes, so a toilet has a stop of its own; Ohio replaced that item of section 606.2 with its own wording, effective October 15, 2025. The supply line that joins that stop to the tank does not come with the toilet: Kohler's sheet says the seat and supply line are sold separately, and American Standard's says fixture only, seat and supply by others.
The rest is layout, and in a small bathroom the layout is the whole argument. The 21 inches in front and the 15 inches at the side decide whether a plan is legal before anyone argues about what looks better, and in a five-by-eight room they usually decide where the shower can go as well. The short version. A toilet usually stays put because the drain, the flange and the vent are already there. The plumbing code sets 15 inches from the center of the bowl to anything beside it and 21 inches of clearance in front, and a building drain serving a water closet is at least three inches. Rough-ins are made at 10, 12 and 14 inches, so the measurement is taken rather than assumed. Federal law has capped a flush at 1.6 gallons since 1994.

Does a toilet have to move when a bathroom is remodeled in Ohio?
Usually not, and the reasons are all under the floor. Does replacing a toilet need a permit in Columbus, Ohio?
Two published rules bear on it. What happens to the toilet flange when a new tile floor goes in?
The floor gets thicker and the flange stays where it was. The plumbing code says the connection is made with a floor flange or a waste connector and sealing gasket, that the flange is attached to the drain and anchored to the structure, and that the joint where the fixture meets the floor is sealed. It says nothing about height. That instruction comes from the makers: Oatey, which manufactures flanges, instructs that the bottom of the top flange be parallel with the finished floor height and sells a spacer kit for the difference, adding that a spacer will not do where the existing flange is broken or badly rusted.
